Medicines reconciliation is an important practice that all healthcare organisations that admit adult inpatients should have policies in place for. Identifying the most accurate list of a patient’s medicines (including name, dosage, frequency, and route) is correct will enable any discrepancies to be recognised and changes documented, thereby resulting in complete list of medications, accurately communicated.
